* Service binder replaces the volunteer fee. The $50 binder will be rolled over to the following year at registration once the necessary hours are performed during the current season. $50 will be refunded at the time of leaving the organization. If no volunteer service is completed during the current year a new $50 deposit will be necessary the following year.

Thank you for providing feedback on our coaches for the 2011 season. Completing this on-line survey is quick, easy, completely confidential and anonymous if you wish it to be. If you would like to electronically sign your feedback, you may do so, however you must choose this option, it is not automatic. The survey focuses on a teams' head coach, however there are questions included to obtain feedback on assistant coaches as well. Please offer any feedback on all coaches as you feel appropriate. Coaches in our program are not paid and dedicate large amounts of personal time and energy to their team and the program, typically at the sacrifice of family and work situations. Please keep this in mind when filling out the survey. Coaches are not perfect and all have areas to improve on. Constructive feedback is deeply appreciated.
